In this study, we will develop and test a digitally supported prevention program to prevent frailty. The program is based on a combination of physical group meetings and supportive welfare technology. A platform for home monitoring and a communication system between the older individual, their relatives, and primary healthcare staff will facilitate communication, support, and preventive measures. The program, including the physical meetings, is based on previous studies conducted by the research team and will be implemented in collaboration with primary healthcare staff.

The purpose of the project is to promote health and participation in daily activities among older adults at risk of frailty, with the long-term goal of improving quality of life and delaying the need for home care services.

Study design and research setting

Based on our previous studies, the preventive intervention to address frailty in older adults, supported by technology, will be adapted, implemented, and evaluated/tested with older adults at risk of frailty who live at home. The project will be connected to Region Stockholm.

The project includes several studies. First, the intervention program will be developed in close collaboration with various stakeholders, including frail older adults, primary healthcare staff, municipal staff, and their managers. A prototype will be tested with 10 older adults at risk of frailty, in close collaboration with primary healthcare. The next step will be to evaluate the feasibility of ACTIVE@HOME in a pilot study and to assess preliminary effects.

Intervention

The intervention group in the sub-studies will be offered a co-created preventive intervention program. The program will include interprofessional measures and group sessions to support an active lifestyle. Additionally, the intervention group will receive a mobile or web application to support self-care and home-based training.
